- [Instructor] Performance form has different sections. Click Route Map. The route map refers to the workflow. In our scenario, the form is first filled by employee and then by manager. Is it also signed by both one by one. Click Employee Information. Here the basic information of an employee, such as his or her name, title and job details are provided. Click Review Date. The review period with appraisal due date can be seen here. Click Goals. All of the goals an employee created for self, he or she was assigned to are visible here, which can be assessed by employee and manager respectively. Rate the goal. Add comments on the goal. Scroll down. Here, additional details on goal can be seen, if there are any. Rate the other goal and scroll down. Rate the goal and scroll down.
