- Data entry don't you just love it? Well, if you don't you can use the forms app to have data for surveys and quizzes captured automatically for you. The first thing you'll need to do is launch the app from the Office 365 portal. There are two types of forms that you can choose from. A form or a quiz. A quiz allows you to input a correct answer and assign a value to a question. A form is used to collect data. We'll start by creating a new form. For our example, Cecilia will be sending out a form to the employees asking for their feedback for the summer picnic. The first thing we need to do is provide a title for our form. And now we can add a question and we have different types of questions that we can ask: a choice, a text, rating, date. I'm going to use a choice.
