- Welcome back incident detectives. In this course we've gone through not only the steps in performing an effective postmortem, but the reasoning why it should be done in a certain way. In the course handout I've provided links to books, papers, blog articles and other sources you can use to delve more deeply into this rich topic. To get started investigating incidents in your own organization just write up guidance on how to perform a postmortem, including blamelessness and education on best practices. Establish a template to use and feel free to use and adapt mine. And lead by example by running some postmortems from document preparation through the postmortem meeting. Practice makes perfect. Many of these points about blamelessness, no root cause and cognitive bias are subtle. Don't get frustrated if it takes your team a little while to get it. Provide patient and calm, but firm and constant pressure on these points…
